2025 Fantasy Outlook
Alcantara spent the year rehabbing from Tommy John surgery undergone in October 2023. The timing of the procedure limited the right-hander to bullpen sessions, though he began to face live hitters in mid-September. All the progress reports were encouraging, but when he takes the hill in the Grapefruit League, it will be more than16 months since Alcantara threw a ball in anger. Typically, the exercises involved in Tommy John rehab strengthen the area, allowing the pitcher to match, and often add, velocity. The concern is always command and control, which were Alcantara's main assets before the injury. Even with a 98-mph fastball, Alcantara's strikeout rate was above average, but not elite. Alcantara did not factor in the decision during Tuesday's loss to Colorado. He allowed two runs on four hits and a walk while striking out four over six innings.

While he couldn't beat the worst team in MLB, Alcantara still snapped an ugly run that lasted over a month. He worked through four shutout frames before the Rockies plated a pair of runs in the fifth inning. Alcantara delivered his first quality start since April 23. He gave up at least four runs in each of his six outings between those two starts, resulting in a brutal 10.08 ERA over 27.2 frames. After three straight outings with at least 100 pitches thrown, he's thrown 71 and 70 pitches in his last two appearances. Still searching for his first win since April 12, Alcantara is expected to make his next start in Pittsburgh.

Alcantara endured a down 2023 campaign with a 4.14 ERA in 28 starts before going down with a UCL sprain in early September, and he underwent Tommy John surgery a month later. The 2022 NL Cy Young Award winner will in all likelihood be sidelined until 2025 given the elbow procedure's typical 12-to-18 month recovery timeline.

The Marlins want to capitalize on Alcantara's value, but it seems his value is declining with each poor start he makes. Teams will likely remain interested in dealing for Alcantara due to the fact that he's under affordable club control through 2027, but the question becomes if the Marlins will still want to deal him this season if they aren't getting a strong return.
